EDWARDSVILLE, IL, November 25, 2025 — Prairie Farms is announcing a recall of select Prairie Farms Gallon Fat Free Milk produced at its Dubuque, Iowa facility and distributed to Woodman’s stores in Illinois and Wisconsin. Some of the product may contain food-grade cleaning agents, which may cause illness if consumed. Only a specific segment of the DEC08 production run was impacted. The affected product was processed between 17:51 and 21:23, representing a portion of that day’s production. To ensure complete containment, Prairie Farms is recalling Gallon Fat Free Milk with the DEC08 code date, PLT19-145 plant code produced during the impacted timeframe of 17:51 to 21:23. On November 24, Prairie Farms was made aware of a potential quality issue. An investigation was immediately initiated, and based on that investigation, affected product was removed from stores and a recall was initiated. Approximately 320 gallons were sold before the issue was discovered. Consumers who purchased the product with the above code dates should not consume it; they may safely dispose of it or return it to the store for a refund. All remaining affected product has been removed from store shelves. MAMTAKIM Inc, located at 888 Magnolia Ave, Elizabeth, NJ 07201, is recalling Item # LL0320 DRIED WHOLE SMELTS SALTED (KORUSHKA), 5.29 oz (150 g) / 20 – 1 box, because this product consists of dried, un eviscerated fish longer than 5 inches, which has the potential to be contaminated with Clostridium botulinum. This bacterium can cause a severe, life-threatening illness or death. Consumers are strongly advised not to consume this product, even if it does not appear spoiled. Botulism, a potentially fatal form of food poisoning, can cause the following symptoms: general weakness, dizziness, double-vision and trouble with speaking or swallowing. Difficulty in breathing, weakness of other muscles, abdominal distension and constipation may also be common symptoms. People experiencing these problems should seek immediate medical attention. The milk in these products is derived from unsalted butter listed in the ingredient statement. People who have an allergy or severe sensitivity to milk risk a serious or life-threatening allergic reaction if they consume these products. Lead is toxic to humans and can affect people of any age or health status, and there is no known safe level of exposure to lead. Even low levels of lead exposure can cause serious health problems, particularly in children and fetuses. Consuming food with elevated lead levels can contribute to elevated levels of lead in the blood. Children and babies are more susceptible to lead toxicity due to their smaller body size, metabolism, and rapid growth. At low levels, children may not have obvious symptoms but can still experience trouble learning, low IQ, and behavior changes. At higher levels of lead exposure, people may experience fatigue, headache, stomach pain, vomiting, or neurologic changes. November 14, 2025, Face Rock Creamery LLC of Bandon, OR is voluntarily recalling 16/6oz. units of Vampire Slayer Garlic Cheddar with a use-by date of 11/04/26 because it has the potential to be contaminated with Listeria monocytogenes, an organism which can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. Although healthy individuals may suffer only short-term symptoms such as high fever, severe headache, stiffness, nausea, abdominal pain and diarrhea, listeria infection can cause miscarriages and stillbirths among pregnant women. We initiate this recall due to the FDA environmental sample identified the presence of Listeria monocytogenes in the processing area where this cheese was packaged. Finished product has not been tested positive for any contaminants. [New York, NY] – ByHeart, a next-generation baby nutrition company, announced today that, out of an abundance of caution, it has chosen to voluntarily recall two batches of ByHeart Whole Nutrition Infant Formula following notification from the U.S. Food and Drug Administration (FDA) of a broader ongoing investigation into a recent outbreak of infant botulism. Infant botulism is a rare but potentially fatal illness that presents a serious threat to the health of infants which occurs when Clostridium botulinum spores are ingested and colonize the intestinal tract, producing botulinum neurotoxins in the immature gut of infants. Affected infants can present with some or all of the following signs and symptoms: constipation, poor feeding, ptosis (drooping eyelid), sluggish pupils, low muscle tone, difficulty sucking and swallowing, weak or altered cry, generalized weakness, respiratory difficulty, and possibly respiratory arrest. ByHeart was notified by the FDA on November 7, 2025 of an estimated 83 cases of infant botulism that were reported nationwide since August 2025. Of these, the FDA also noted that 13 infants received ByHeart formula at some point. The FDA has not identified a direct link between any infant formula and these cases and there is no historical precedent of infant formula causing infant botulism. Botulism is extremely uncommon in dairy products or infant formula, and is naturally occurring in environmental sources like soil, select vegetables, and dust.

Nov. 6,2025 – LAKE ZURICH, Ill.— Fresenius Kabi, part of the global healthcare company Fresenius, and a leading provider of essential medicines and medical technologies is voluntarily recalling three lots (numbers 6133156, 6133194, 6133388) of Famotidine Injection, USP, 20 mg per 2 mL (10 mg per mL), 2 mL Fill in a 2 mL vial. This recall is being performed to the user level in the United States. The product is being recalled due to out-of-specification (OOS) endotoxin results of certain reserve samples from a single lot. Based upon the investigation, two additional lots were also included in the recall as a precautionary measure. Elevated endotoxin levels can precipitate severe systemic reactions such as sepsis and septic shock. Severe responses may include inflammatory and life-threatening immune responses and death. Non-serious adverse event reports potentially associated with the OOS have been received for one lot. These non-serious adverse events included chills, change in mental status, change in respiratory status, fever, increase in body temperature, shivering and shaking. To date, no adverse event reports have been received for the second and third lots.

All Organic Moringa Leaf Powder purchased after 06/05/2025 is included in this recall. As of today (November 6, 2025), we have not been informed of any illness from the Moringa powder sold in this lot.
